We are dedicated to developing sustainable models for Community Health Worker (CHW) services. We connect patients with certificate-holding Community Health Workers to improve health outcomes.

Our Story

CHW Solutions was founded in St. Paul, Minnesota by Megan Ellingson and Megan Nieto in 2016. “The Megans” first teamed up in 2001 to screen for and prevent lead poisoning in Minneapolis children and pregnant women. That work increased lead screening rates from 50% to 80% among children receiving Medicaid. As home visitors and CHWs themselves – serving young children, families, individuals, and older adults – the Megans have experienced the life-changing spark between CHWs and their patients. At CHW Solutions, they combine their unique skills to improve health in the communities they serve.

The philosophy behind CHW Solutions is to bring gold standard CHW care to patients, driven by compassion and fueled by a diverse workforce. We meet patients where they are, and value the roads their lives have taken. Every day, our CHWs are revolutionizing the circle of care for patients, working with clinical teams, public health, government, and health plans to assure CHW services are available to those who need them.

Megan Nieto (CHW)

Megan N.’s (she/her/ella) expertise is in direct supportive services for families facing lead poisoning, healthy homes, asthma, and other chronic health conditions. Megan’s professional experience includes serving as the Director of Community Health Programs at a community-based organization and as a Community Lead Educator. Supportive services expertise includes in-home visiting, resource referral and connection, clinical lead testing services, and case management. In these roles, Megan developed contracts with health insurance companies, and low-cost mechanisms for processing claims, so her organization could supplement its grant-funded efforts with insurance payment income. Megan has a Bachelor of Science degree in Biology from St. Cloud State University and is bilingual in Spanish/English. She also received her Community Health Worker certificate in 2017 from Northwest Technical College in Bemidji. Megan currently teaches the Minnesota CHW Certificate courses — in English and Spanish — as an online instructor at St. Mary’s University in Minnesota.

Cathy (she/her) has an extensive background in the field of human services, including working with non-profit organizations, for-profit health care companies, and state agencies.  Cathy has worked with adults living in long term care facilities, community settings and as a health educator.  Cathy developed training materials and facilitated training sessions for a national long term care company, and developed educational materials for community members during her employment at the Minnesota Department of Health. Cathy has also been employed as a grant-funded contractor with the University of Minnesota focusing on workplace safety.

Cathy received her Community Health Worker Navigator certificate in 2021 from Normandale Community College in Bloomington, Minnesota.  Cathy has a Bachelor of Science degree from the University of Minnesota, Mankato specializing in Therapeutic Recreation and Gerontology.  Cathy received a Master of Science Degree from Florida State University in Tallahassee, Florida with a focus on Therapeutic Recreation and Rehabilitative Counseling.

I enjoy communicating with people of different cultures and ages, and I believe that through communication, mutual understanding and mutual respect, we improve our lives and the lives of our loved ones.

Macaran (Mac) Baird (he/him) is a graduate of Macalester College and the University of Minnesota Medical School. He started in rural practice in Wabasha, MN and then had an academic career in Oklahoma, New York State and then back to Minnesota where he retired after 15 years as Head of the Department of Family Medicine and Community Health. He currently serves on the Board of the Hiawatha Valley Mental Health Center and is a volunteer physician at the CARE Clinic, a free clinic in Red Wing, MN. He enjoys sailing, kayaking and fly fishing.
