Medica is a proud partner of CHW Solutions to provide members with a sustainable model of Community Health Worker (CHW) health education since 2021.  Medica is one of seven health plans that today provide integrated Medicare and Medical Assistance services in the Minnesota Senior Health Options (MSHO) program for people age 65 years and over; Medica is also among the three original health plans when MSHO began in 1997.  In 2020 Medica sought an evidence-based health coaching program that meets our MSHO members where they live in communities across Minnesota.  CHW Solutions proved themselves the best fit to partner with Medica based on their robust service delivery and culturally capable presence within Minnesota communities.  The COVID-19 public health emergency posed additional implementation challenges that CHW Solutions superbly navigated through their flexible application of in-person, virtual and telephonic visits.”

Project HEALINGS & Diversity Council

COVID-19 Vaccine Encouragement Project

Project HEALINGS (Health Education and Adult Learning Incorporated with Navigating Government Systems) was designed as a service hub for entities serving Minnesotans disenfranchised by identity and circumstance during the COVID 19 pandemic. Convened by Diversity Council, HEALINGS serves diverse communities across our state to focus on COVID 19 vaccine access and messaging around culturally specific hesitancies and barriers. CHW Solutions partnered with HEALINGS teams, providing general outreach and education contacts to individuals served by their staff throughout Minnesota. CHW Solutions developed and shared trainings in support of the efforts of community health workers supporting communities seeking health equity. Project HEALINGS teams benefited greatly from CHW Solutions efforts to support our efforts for COVID response as well as professional workforce development.

Cook County

Public Health and Human Services

Cook County Public Health and Human Services had a great experience working with CHW Solutions to help us create a sustainable model to offer peer support to childbearing families during the first year of the COVID-19 pandemic.  They helped us develop a roadmap for how to implement our vision that led to two community members becoming certified as Community Health Workers through an online program at Northwest Technical College.  The ideas we worked on with CHW Solutions are still alive and well as we continue to explore how to expand CHW services to people who are at increased risk of isolation and poor health outcomes in our rural community.

The Minneapolis Health Department has been delighted to partner with CHW Solutions on multiple projects to connect patients living with chronic diseases, such as diabetes and heart disease, to community health workers (CHWs) who can provide health education and support with social needs. CHW Solutions staff have helped to identify innovative strategies to integrate CHWs into regular medical care. Examples include working with community clinics to develop patient referral systems, as well as integrating CHWs into a clinic. CHW Solutions staff are always willing to try new ideas while also staying focused on patient needs. The support that CHW Solutions staff provides helps people better manage their disease and live a healthier life.

Megan Ellingson (CHW, MHA)

Megan E.’s (she/her) expertise is in collaborative planning and implementation to address health issues requiring both clinic and community approaches. Megan’s professional experience includes serving as a Health Policy and Program Coordinator at the Minneapolis Health Department, and as a Department Manager in women’s services and pastoral care at Intermountain Healthcare. Her direct patient care experience includes serving as a Patient Advocate at MayView Community Health Center, as a Care Guide in late-life supportive care at Allina Health, and as a hospice volunteer with her therapy cat, Sweet Pea, at HealthPartners. Megan has a Bachelor’s Degree in Human Biology and Medical Anthropology from Stanford University, and a Master’s Degree in Healthcare Administration and Financial Management from the University of North Carolina, Chapel Hill. She also obtained her Minnesota CHW certificate from Northwest Technical College in 2018. Megan currently teaches the Minnesota CHW Certificate courses as an online instructor at St. Mary’s University in Minnesota.
