One Afghan Community Health Worker Trainee position available.

Under a contract with the Minnesota Department of Human Services – Resettlement Program Office (DHS-RPO), CHW Solutions is establishing CHW services for refugees and other Minnesota newcomers under humanitarian protection. We are hiring one (1) Afghan CHW Trainee to join our team of newcomers. In addition to providing direct home visiting services, the CHW Trainee will take classes and complete an internship to obtain their Minnesota Community Health Worker certificates. The CHW Trainee will spend approximately 50% of their time providing direct CHW services and 50% of their time in classes/training.

CHW classes will be provided through one of the Minnesota colleges or universities delivering them. Full scholarships are available to CHW Solutions’ CHW trainees.

Applicants must be highly motivated, have excellent communication skills, be able to work independently, and be able to create and maintain positive relationships with our CHW team, patients, management, executive leadership, and partners.

The CHW Trainee position is full-time (32-40 hours per week) and will play a major role in implementing our company’s vision for CHW services. Applicants should be comfortable with a high level of responsibility and be energized by serving diverse communities and being a part of our company’s expansion.

This position will work closely with the company’s Lead CHWs to achieve desired outcomes.

Responsibilities:

Key responsibilities of the role include:

  • Establishing and managing a schedule of face-to-face home visits (and telehealth visits as needed) with the designated patient population.
  • Providing direct Community Health Worker services in patients’ homes, in the community at locations familiar to and comfortable for patients, or telehealth visits (as needed). CHW services will include health education, self-management skill building, and connecting to community resources (nutrition, transportation, primary healthcare, financial, social support, etc.).
  • Helping patients and their families navigate systems and technology to access needed services and resources.
  • Delivering services in a dignified and respectful manner that empowers patients to have ownership over their own health and confidently navigate relevant systems, programs and community resources.
  • Identifying the root causes of patients’ barriers and challenges and use creativity and collaboration to solve problems when necessary.
  • Timely and accurate collection and documentation of relevant data for the purposes of billing payors, tracking performance and identifying gaps in care.
  • Active participation in team huddles/meetings, trainings, health coaching sessions, and other partner meetings as requested.

Requirements:

  • United state’s high school diploma or GED (or demonstrate equivalent from other country)
  • English skills sufficient to pass the Duolingo test (score 85 or higher)
  • Ability to conduct home visits
  • Willingness to participate in background checks and the ability to meet CHW Solutions’ employment criteria
  • Excellent communication skills with patients, co-workers, and partners
  • Ability to use technology to conduct research, connect clients to resources, conduct virtual visits and record and report clinical and administrative activities
  • Physical requirements may require climbing, balancing, stooping, reaching, standing, walking, grasping, talking, hearing and seeing. The role may also require visiting patients in settings that are not handicap accessible.
  • Ability to achieve operational and performance goals on a consistent basis.

Preferred Qualifications:

  • Fluency in the following languages: Dari and Pashto
  • Prior work in health care delivery, public health, social services, education, coaching or other relevant fields

Megan has a Bachelor’s Degree in Human Biology and Medical Anthropology from Stanford University, and a Master’s Degree in Healthcare Administration and Financial Management from the University of North Carolina, Chapel Hill. She also obtained her Minnesota CHW certificate from Northwest Technical College in 2018. Megan currently teaches the Minnesota CHW Certificate courses as an online instructor at St. Mary’s University in Minnesota.
