Part-Time Community Health Workers (CHWs) – Greater Minnesota

Role Overview: We are seeking two part-time extremely responsible Community Health Workers (CHWs) who are passionate about serving Minnesota’s immigrant and refugee communities. This role involves delivering health education and outreach topics in 1:1 and group settings for clients under a contract with the Minnesota Department of Human Services – Resettlement Programs Office (DHS-RPO). CHWs will conduct visits in patients’ homes and in community settings.

Key Responsibilities

  • Deliver health education and outreach topics in 1:1 and group settings to RPO clients in Greater Minnesota (outside of the Twin Cities Metro Area).
  • Work with diverse DHS-RPO CHW team and RPO-funded community providers.
  • Deliver health promotion topics (health literacy, health navigation, health education and outreach, self-management skill building, connecting to community resources) in culturally and linguistically relevant ways.
  • Address emerging community needs and specific health topics (e.g., preventive care, healthy lifestyles, managing health conditions, stress reduction).
  • Develop professional relationships and connections with health care teams and community resource providers outside of the Twin Cities Metro Area.
  • Collaborate with other CHWs on the team to assure all patients statewide have access to CHW home visiting services.

Additional Responsibilities:

  • Coordinate with other CHWs on the team, interpreters, and other cultural representatives.
  • Work with RPO network community providers to assure people living in Greater Minnesota who are here under humanitarian protection receive health promotion services.
  • Assist company Co-Founders, CHW Services Director, and Lead CHW to refine existing health education practices to develop models of patient care that best serve patients living in Greater Minnesota.

Qualifications:

  • Excellent communication skills with patients, co-workers, and partners.
  • Experience delivering health education and outreach services.
  • Driver’s License and reliable vehicle.
  • Experience working with refugees and other newcomers to Minnesota.

Preferred Qualifications:

  • CHW certificate from one of the Minnesota Community Health Worker training programs. (CHW Solutions will consider supporting applicants to obtain their CHW certificate while employed at CHW Solutions.)
  • At least one year of direct patient contact as a CHW or similar role.
  • Experience documenting electronic health records and following HIPAA and other security procedures.

Megan E.’s (she/her) expertise is in collaborative planning and implementation to address health issues requiring both clinic and community approaches. Megan’s professional experience includes serving as a Health Policy and Program Coordinator at the Minneapolis Health Department, and as a Department Manager in women’s services and pastoral care at Intermountain Healthcare. Her direct patient care experience includes serving as a Patient Advocate at MayView Community Health Center, as a Care Guide in late-life supportive care at Allina Health, and as a hospice volunteer with her therapy cat, Sweet Pea, at HealthPartners. Megan has a Bachelor’s Degree in Human Biology and Medical Anthropology from Stanford University, and a Master’s Degree in Healthcare Administration and Financial Management from the University of North Carolina, Chapel Hill. She also obtained her Minnesota CHW certificate from Northwest Technical College in 2018. Megan currently teaches the Minnesota CHW Certificate courses as an online instructor at St. Mary’s University in Minnesota.
